Maksim Birikov, DPT The Practitioner Behind Physica Medica

I'm Maksim Birikov, Baltimore's first Wim Hof certified instructor and the only practitioner offering Pancafit in the United States. At Physica Medica, every session is one-on-one with me: I do the assessment, I do the treatment, I build the plan. Nothing gets lost in translation, because there's no one to translate to.

The Foundation

Credentials & Training You Won't Find at Another Baltimore Clinic

01

Doctor of Physical Therapy

The clinical foundation for everything else. A DPT means doctoral-level training in diagnosis, anatomy, and rehabilitation. So when I put a needle, a tool, or a hand on your body, I know exactly which structure I'm treating and why.

02

First Certified Wim Hof Instructor in Baltimore

Breathwork and cold exposure with a real physiological mechanism: down-regulating the nervous system where chronic pain and tension live. No other Baltimore PT can integrate it into treatment, because no other Baltimore PT is certified to teach it.

03

The Only Pancafit Class in the US

An Italian method of global postural realignment I trained in Europe to bring here. For you, it means a tool for posture and chronic tightness that literally does not exist anywhere else in the country.

04

Scoliosis Correction Without Surgery or Braces

Most PTs refer scoliosis out. I treat it with a structured, movement-based program that measurably improves alignment. It's the third option patients are rarely told exists.

The Model

The Philosophy: Why One-on-One, Every Time

The one-on-one model isn't a business limitation. It's a clinical decision. When one practitioner handles assessment, hands-on treatment, and movement programming, nothing gets lost between providers. I remember what your tissue felt like last week. I see whether the correction held. I adjust in real time instead of reading someone else's notes.

That continuity is why results here come faster and hold longer. Most clinics silo manual therapy from corrective exercise: a massage here, a printout of exercises there. I integrate both in the same hour: release the restriction, then immediately retrain the pattern that created it. Releasing a trigger point without retraining the movement is a two-day fix, and I'm not interested in two-day fixes.

It also means I'm selective. I take fewer patients and give each one a full hour, every visit. If you want volume, the chains are expanding aggressively. If you want the problem fixed, you know where I am.

The Lineage

European Techniques. Baltimore Practice.

American PT education is strong on protocol and weak on fascia. My approach draws on European manual therapy traditions that treat the body as one connected system. Pancafit is Italian: global decompensated stretching that releases the entire fascial chain instead of one muscle at a time. The Wim Hof Method is Dutch, breathwork and cold exposure that trains the nervous system itself.

What that means for you is practical, not academic: when your hunched back hasn't responded to years of stretching, or your chronic tension keeps returning after every massage, it's usually because the treatment was local and the problem is global. The European toolkit exists precisely for those cases, and in Baltimore, this is the only place to find it.

About Maksim & the Practice

Doctor of Physical Therapy (DPT), Baltimore's first certified Wim Hof Method instructor, the only Pancafit practitioner in the United States, and specialized training in non-surgical scoliosis correction and European fascial rehabilitation techniques.

Because continuity is clinical. One practitioner doing assessment, treatment, and programming means faster diagnosis, treatment that adjusts in real time, and results that hold. It's the opposite of the rotating-therapist model at every chain clinic in Baltimore. That's why patients choose me by name. Learn what a session looks like on the first visit page.

Structured breathing exercises and cold exposure that measurably shift nervous system regulation. Chronic pain and chronic tension are nervous system states as much as tissue states, which is exactly why a physical therapist should teach it. I integrate the breathwork directly into rehabilitation rather than selling it as a wellness add-on. More on the Wim Hof Method page.
